We have made several of our forms available for download in order to expedite the check-in process during your first visit. Please complete the Patient Data Sheet and the Financial Policy beforehand and bring the completed forms with you to your appointment.

Frequently Asked Questions

Do I need a referral from my doctor to be seen by a physical therapist?

All physical therapists in Oregon are licensed to provide physical therapy without referral from a physician. The law states that treatment may continue for a period of up to 30 days without physician referral. However, a physician’s referral must be obtained in order to continue treatment past the initial 30 days. It is important to check with your insurance to determine if a physician referral to physical therapy is required. Not all plans will provide physical therapy benefits in the absence of a physician referral.

Is my insurance accepted at Pettygrove Physical Therapy & Sports Rehabilitation?

There is a complete list of insurances for which we are in-network providers. If your insurance company is not listed, please feel free to call our office to determine if therapy in our office will be financially sensible for you.

How do I know what my insurance covers?

We rely on you to provide us with accurate and up-to-date information regarding your insurance. As a courtesy to you we will contact your insurance company so that you will be aware of any out-of-pocket expenses that you may incur. These expenses include co-pays (due at each therapy appointment), co-insurances, deductibles, and certain durable medical supplies. We encourage you to also contact your insurance company to verify your benefits. Your insurance company will be billed by our office each day following your therapy appointment so that you do not have the extra stress of submitting the claims yourself.

How do I schedule an appointment?

You may contact our office by phone at (503) 228-1306 during normal office hours (Monday to Thursday, 7 am to 6 pm; Friday, 7 am to 5 pm). To schedule the initial visit we will need some general information that may be obtained over the phone. This information allows us to inform you of the expected insurance benefits, and to ensure immediate and proper billing to your insurance company. Follow-up visits with the physical therapist are typically scheduled after your initial evaluation so that the proper frequency may be determined.

What if I need to change my appointment?

We want to accommodate our patients as best as possible, and we expect that appointments will need to be changed from time to time. Please be aware that your therapist may not always have open appointment times to accommodate your exact scheduling needs, especially with short notice. For that reason our therapists work in teams of two in order to provide you with scheduling options while maintaining consistency of your therapy. If you need to cancel an appointment we ask that you do so with at least 24 hours notice (a $25 fee may apply with cancellations less than 24 hours notice). We always appreciate a phone call when you need to cancel or if you are running late. A simple, quick call can allow us to provide the best possible service to you and to our other patients.

What should I expect on my first visit?

Please plan on arriving 15 minutes prior to your scheduled appointment time in order to complete information forms. You should allow between an hour and an hour and a half for the physical therapist to thoroughly evaluate you and begin your treatment. You and the therapist will discuss your symptoms and any limitations with activity that exist, and he/she will perform special tests or measurements as necessary to aid in determining the best course of treatment. Our therapists emphasize educating you regarding your condition because a well-informed individual will be able to better care for their body. You may also receive instruction in specific exercises or activities tailored to your individual condition to be performed at home to give you further control in your rehabilitation. Our therapists maintain trusting relationships built on communication with many of our referring physicians. This means that in many cases your physical therapist and physician will openly discuss your condition in order to determine the best course of action to reach your rehabilitation goals.

What do I need to bring to my first visit?

We will need you to complete information forms prior to beginning your appointment with the therapist, so we suggest arriving 15 minutes prior to your scheduled appointment time. To save you time you may download the necessary forms located at the top of this page and bring them completed to your first visit. It is essential to bring your insurance card and the doctor’s referral for physical therapy so that treatment may begin.

What should I wear to my therapy appointment?

Please wear or bring clothing that will allow examination of your injured area. For example, shorts or loose-fitting exercise pants are helpful in examining your hip, knee, ankle, or low back. If we will need to view your shoulder, neck, or upper back it will be helpful for women to wear a sports bra or short-sleeve t-shirt, and for men to wear a short-sleeve t-shirt or be willing to remove the shirt. Men’s and women’s changing areas with lockers are provided, as well as access to a shower.

How often do I need therapy?

Patients are typically seen by the therapist one to three times a week. Your therapist will give you as much information as possible regarding the time frame and progression of your rehabilitation. The frequency and duration of therapy is based on input from your physician, your physical therapist, and – most importantly – from you. It is important to remember that your progress is influenced by a number of factors – some can be controlled and some can not. In general, progress is helped by limiting daily activities that are painful and by minimizing inflammation of the involved muscle, tendon, or joint.

Is parking available at your clinic?

We are located on the fourth floor of the Northwest Center for Orthopedics & Rehabilitation. There is a parking garage located at ground level with access from Quimby. Additional parking can be found in a lot with access off of 18th Avenue (just north of Raleigh). At this time street parking is not metered, and only zoned for time directly in front of our office on 18th Avenue.  You can download printable directions and a map.
